ADVANTAGE Health Solutions, Inc.^SM is proud to be selected by the Indiana Family and Social Services Administration, Division of Aging, to provide transitional services for an initiative called Money Follows the Person. The primary goal of Money Follows the Person is to transform the model of long-term care throughout the State by giving residents in nursing facilities the opportunity to live in their own home or community-based setting.
