Disease Management As part of our mission to serve the older adult populations of Indiana, we offer services that go beyond traditional Medicare coverage. We work to ensure our members have the best quality of life possible. Living with a chronic disease can be challenging. We strive to partner with our members at whatever stage of their disease process with resources such as self management materials, outreach calls or provider education. ADVANTAGE works to provide additional support to our members for the following conditions:
We also offer a 24-hour nurse call line for additional support to our members. This Clinical Support Service is made to available to all ADVANTAGE Preferred Plus Members. Wherever our members are in their stage of life, our goal is to provide needed resources aimed at keeping our members active and healthy.
