SNP Provider Training Materials
It is a requirement of the SNP Model of Care that all providers who have contractually agreed to be SNP providers with ADVANTAGE Health Solutions, Inc., complete this training. Likewise, providers can elect a member of their clinical staff to complete the training. After completion of the training, you must return the attestation to validate your participation. The link to the attestation form can be found at the end of the SNP training PowerPoint.
