Members may be identified to participate based on overall health and pharmacy needs.
ADVANTAGE Health Solutions works to partner with the member, provider, and pharmacy.
The goal of the program is to optimize medication use based on clinical conditions.
The ADVANTAGE Health Solutions Patient-Centered Medical Home (PCMH) Program is created to improve care management support and clinical outcomes of members enrolled in ADVANTAGE Health Solutions. Our PCMH program targets those members/patients who have been diagnosed with conditions that may require more intense care needs. Through providing enhanced reimbursement and data exchange, PCMH providers are given the financial resources to develop and manage a comprehensive care plan for each participating member/patient.
In addition, the ADVANTAGE Health Solutions Transitional Care Team plays a role in assisting both the PCMH provider and the participating member/patient with coordinating medical needs and reducing worries. The Care Team is able to help members improve their ability to manage their care. For example, our Team provides support when members are recovering from an illness, education about medicines or help in talking with their doctors and finding community resources. We are here to help our members, your patients, navigate the health care system.
