At ADVANTAGE Health Solutions our goal is to give our Medicare members clinical support to improve care. We offer services that go beyond traditional Medicare coverage.

Our Care-ADVANTAGE programs assist in coordinating the care of our members and reducing their worries. The Care Team is available to address health concerns and help by arranging services. We give education about medical conditions or medicines and assist members in talking with their doctors. We are here to help our members manage their care and navigate the health care system.

Our Care Management team consists of:

 • Advanced Practice Nurse (Nurse Practitioners)

 • Nurses

 • Health Coaches

 • Social Workers

 • Pharmacist

 • Medical Director (Licensed Physician)
