You can join ADVANTAGE Health Solutions if:
- You are entitled to Medicare Part A and enrolled in Medicare Part B.
- You live in the service area.
- Individuals with End Stage Renal Disease are generally not eligible to enroll in
ADVANTAGE Health Solutions unless they are members of our organization and have
been since their dialysis began.
- You must continue to pay your Medicare Part B premium.
- For individuals enrolling in the ADVANTAGE Special Needs Plan (SNP), you must currently have both Medicare and Medicaid benefits.
Individuals may only enroll in our plans during certain times of the year. Please
contact member services for more information.
How to Enroll
Option 1: Online Enrollment
Enroll in a PPO or HMO Plan online beginning October 15, 2011 click here
(Online enrollment in Special Needs Plan is not available at this time)
Option 2: Paper Application
and then do one of the following:
- Provide the completed application to your agent
- Mail the completed application to:

- Fax the completed application to 1-800-906-5461
Option 3: CMS Online Enrollment
Medicare beneficiaries may enroll in ADVANTAGE Health Solutions through the CMS
Medicare Online Enrollment Center located at www.Medicare.gov.
ADVANTAGE Health Solutions will send a confirmation letter upon confirmation of
enrollment by the Centers for Medicare and Medicaid Services (CMS). This letter
follows the welcome packet.
New members will receive a welcome packet within 7 business days. The welcome packet
includes the following:
- Welcome letter
- Identification cards
- Evidence of Coverage
- Rx mail order information (if applicable)
- Pharmacy Directory
- Formulary Listing
- Provider Directory
If you decide to change the way you pay your Plan premium, it may take up to three
months for the change to take place.
