ESTABLISHING A MEDICAL HOME

Your doctor or PMP is your “medical home.” We call it a medical home because the health care providers in that office know you and care about you. They will help you with your health care needs.

When you first become an ADVANTAGE Care Select member, call your doctor to make an appointment, even if you are not sick. We want you to see your doctor within the first 60 days of joining our health plan. This is a very good way for you and your PMP to get to know each other and for your PMP to see what your health needs are. Your care manager will remind you to do this when they do your screening, if you haven’t made your appointment already.
