CARE MANAGEMENT

ADVANTAGE Care Select's Care Management program provides a wide spectrum of services for members, families and providers who need assistance in finding and using appropriate health care and community resources.

The ADVANTAGE Care Select Care Management staff considers members' medical, social and cultural needs in targeting, assessing, monitoring and implementing services for them. The Care Management staff also helps members and families find their way through the complex medical and behavioral health care systems.

Members with specific diseases such as asthma, diabetes, chronic obstructive pulmonary disease (COPD) and congestive heart failure (CHF) are eligible for disease management services through our Care Management program.

Care Management services available to ADVANTAGE Care Select members may depend on the member's age and diagnosis. Check the Member Handbook or call Member Services at 1-800-784-3981.
