ADVANTAGE Care Select helps members with developmental disabilities. Our goal is to help you take care of your disability so that you feel better now and in the future.
We help members that have more than one physical or developmental issue.
» People that cannot take care of themselves or live alone.
» People that have trouble speaking.
» People that have trouble learning.
» People that have trouble getting around.
Watching Your Health
Taking care of your health will help you feel better every day. We give you support and help you find extra care when you need it. We can help you find a doctor, schedule appointments and make a plan.
We work with you, your family and your doctor.
» We help you figure out what you need.
» We make a health care plan and share it with other people who need to know.
» We help you follow the plan.
» We help you find programs in the community.
» We keep in touch with you during the year to see how your plan is working.
