Aged, Blind, Disabled
ADVANTAGE Care Select helps members that are Aged, Blind or Disabled. Our
goal is to help you take care of your health and feel better now and in the future.
We help people that are:
» Legally blind or have a major loss of vision
» Unable to work because of a physical or mental problem
Watching Your Health
Taking care of your health will help you feel better every day. We give you support
and help you find extra care when you need it. We can help you find a doctor, schedule
appointments and make a plan.
We work with you, your family and your doctor.
» We help you figure out what you need.
» We make a health care plan and share it with other people who need to
know.
» We help you follow the plan.
» We help you find programs in the community.
» We keep in touch with you during the year to see how your plan is working.
How You Get Help
If you want help, contact your Disease Manager. If you do not have a Disease Manager or
need to find out who your Disease Manager is, contact us at (800) 784-3981.
Caregiver Checklist
If you are a caregiver, these are warning signs for those members that might benefit
from these services:
» Many emergency room visits
» Many hospital admissions or readmissions
» The member does not go to the doctor for a serious or ongoing medical
problem
» The member has a psychiatric/behavioral health condition and is not
going to a psychiatrist/mental health professional
» Many missed doctor’s appointments
» Not taking medications or following a treatment plan
» The member does not know about community programs that may help them
» The member is not interested in taking care of their health
