ADVANTAGE HMO is our most popular health insurance option chosen by both small and large group employers. Our HMO Plans emphasize health and wellness to promote healthy lifestyles and improved health outcomes while managing medical costs. The HMO Plans include primary care physicians from a broad network of Physician Hospital Organizations (PHOs).

Plan Features:

• Employees and their dependents can choose different Primary Care Physicians (PCPs)
• Member chooses a Primary Care Physician (PCP)
• Member pays a predictable copayment or coinsurance for in-network services
• Members can go to most in-network specialists without a PCP referral
• PCPs are available to help navigate the health care system
• Disease Management and complex case management are included without additional cost
• Members can enroll in the Mountain of Wellness program
• Employers can actively engage in benefit plan designing

ADVANTAGE Point of Service (POS) Benefit Plan

ADVANTAGE HMO and POS Plans offer employers and their employees the flexibility of a traditional insurance plan but with cost containment features of a managed care plan.

Plan Features:

• Members choose a PCP and receive full benefits in-network
• In-network benefits are available for copayment or coinsurance
• Members can go to most in-network specialists without a PCP referral
• Members have the flexibility to access out-of-network providers without a referral, but must pay higher out of pocket expense
• PCPs are available to help navigate the health care system
• Disease Management and complex case management are included without additional cost
• Members can enroll in the Mountain of Wellness program
• Employers can actively engage in benefit plan designing
