As an ADVANTAGE member all of your medical benefits (except for emergency services, urgently needed services and some specific preventive services) are provided and arranged by your Primary Care Physician (PCP), a personal physician you choose from our directories.

The following is a list of participating provider networks. To find a provider using our online search, please visit our Find a Doctor page.

•  Franciscan Directory - Central Indiana Region Franciscan Alliance employees of St. Francis Health, Physician Network CIR, ABO CIR, Solutions and SFHN MD Offices Only.
•  Franciscan Alliance Directory - Self Funded Network for Franciscan Alliance employees of all Northern and Western Indiana Region hospital facilities; including Physician Network, Information Services, CBO, ABO, Omni Health & Fitness, Hammond Clinic, Pharmacy and Corporate employees.

• PHCS Directory - Employer Groups that have elected to cover employees that reside outside of the service area or an Out-of-Area Plan; Franciscan Alliance members enrolled in the Indiana Open Access Plan; Employer Groups that offer the ADVANTAGE Choice Product that allows members residing in the service area to elect to see providers in the PHCS Network.
