IMPORTANT: If you receive services at an IU Health facility, your claims will be paid at your Tier 2 benefits level. IU Health facilities are NOT Tier 1 hospitals providers.

For Tier 1 and Tier 2 benefits clarification, please refer to your member benefits booklet distributed earlier by the Indiana Teamster Health Benefits Fund.

If your doctor is currently not participating in the network, you may nominate your physician for consideration. By completing the Physician Nomination Form and following the instructions therein, the form will be automatically sent to ADVANTAGE for review. We will make every effort to follow-up on your request and respond back to you as soon as possible.
